This is a rare presentation of a primigravida at 27 weeks gestational age who presented with signs and symptoms of acute intestinal obstruction and inability perceive fetal movements for one day. Past medical history of Rheumatoid arthritis treated with sulfasalazine and Psoriasis treated with 'Siddha' medicine. Uneventful antenatal period. Plain Xray of the abdomen showed a large distended loop of bowel in the upper abdomen and Ultrasound of the abdomen showed moderate ascitis in the perisplenic region and a gravid uterus with a single live intrauterine gestation. With the clinical suspicion of acute intestinal obstruction patient was taken up for Laparotomy.
