BACKGROUND : Twin reversed arterial perfusion is a rare but serious complication of monozygotic monochorionic gestation (incidence 1 in 35,000 births). In the TRAP SYNDROME there is a normally formed donor twin with features of heart failure ; nd a recipient twin without heart. One such interesting case is presented below. CASE : A 24yearold Primigravida was admitted in our unit at 29 weeks of gestation with fundal height corresponding to 34 weeks. Initial USG at 22 weeks showed one twin of 24 weeks gestational age and the second with lowci ' of spine, abdomen, and a huge cystic mass around the spine without head and thorax.
