True double vagina includes a double introitus and resembles double-barreled shotgun, each passage terminating into distinct, separate cervix and uterus. Mrs. S, aged 20 years, a primigravida with 36 weeks of gestation with history of labor pains, P/V leak since 12 hrs was admitted. Pelvic examination revealed a complete longitudinal vaginal septum, two cervices. Right cervix was dilated 3-4 cms and breech was at -3 station. Left cervix was uneffaced and undilated. Patient had O Rh negative blood group. An emergency cesarean section was performed.
