Twin pregnancy in association with rudimentary horn pregnancy is extremely rare. Rudimentary hom pregnancy coexistent with an intrauterine pregnancy usually results in rupture of the rudimentary horn and death of the correspondent twin. We present an unusual case of twin pregnancy with one of the fetuses in rudimentary horn. A 28 years old G2P1L1 with 5 months amenorrhoea was admitted with features of rupture uterus. At laparotomy, gross haemoperitoneum was seen. Right-sided non-communicating rudimentary horn was found to be ruptured. A dead male fetus found lying in peritoneal cavity was removed. Placenta and membranes were densely adherent inside the horn.
