To demonstrate a case of pyocolpos in 23yr old unmarried sexually inactive regularly menstruating girl with sepsis. Design: A single case study of a rare case. Settings: Teaching corporate hospital. Material & Methods : An unmarried 23 yrs old female presented with acute pain in abdomen and headache since 2 days. She had recurrent attacks of UTI from the last 2 months. She had undergone vaginoplasty 8 yrs ago for cryptomenorrhea. She has been having regular menstrual cycles with moderate flow. On examination she was having tachycardia and hypotension, with cold peripheral extremities .local examination revealed stenosed vaginal introitus with normal external genitalia.
