A 34 year old of woman with severe mitral stenosis with moderate tricuspid regurgitation and pulmonary artery hypertension grade IV NYHA came for emergency cesarean section with 38 weeks of gestation with fetal distress. Mitral stenosis is the most common of the lethal cardiac lesion occurring in pregnancy and is present in 0.3-1.2% of parturient. Here combined spinal epidural anaesthesia was planned and patient was taken for surgery, colloid infusion started slowly . 2.5 mg 0.5% bupivacaine and 25µg fentanyl was given through spinal route and 50mg of 2% plain xylocaine diluted in 5 ml was given through epidural catheter slowly after aspiration.
