A 60-yr-old male with right renal carcinoma scheduled for right radical nephrectomy required internal jugular (IJV) cannulation. The IJV was punctured using #18G needle, under US guidance. The color of blood aspirated and lack of pulsatile flow on syringe disconnection suggested venous origin. Further cannulation was done by Seldinger technique. Subsequently pressure waveform monitoring revealed carotid artery cannulation. Since IJV cannulation done under US guidance we decided to go for check US scan. This scan revealed central venous catheter (CVC) traversing IJV before entering carotid artery.
