Patient with hereditary coagulopathies and intracranial bleeds have high mortality and need emergency surgical intervention. Immediate diagnosis and early treatment with replacement therapy with definitive neurosurgical intervention may change the outcome. We report a case of spontaneous subdural haematoma in an elderly hemophiliac. Case report: A sixty-year-old male was diagnosed fronto- temporal- parietal SDH with mid line shift. He had hemoglobin of 6.8 gm/dL with a normal bleeding, clotting time, thrombin time and prothrombin time . Platelets count was 1.5 lakhs/mm3. aPTT was 58.5/30. Emergency craniotomy was planned for evacuation of haematoma.
