Introduction: Auto immune hemolytic anaemia is a rare disease which is even rarer in pregnancy. It is caused by IgM auto antibodies directed against the polysaccharide antigen located on the RBC surface. The annual incidence of auto immune hemolytic anaemia is one in 80,000. This case presents the cliallenge surrounding the management of cold auto immune hemolytic anaemia pregnancy Case: 22 years married since 4 years G2 PI Dl 35 weeks pregnancy with previous LSCS with IUFD with rupture uterus with severe anaemia of Hb of 3gm%. Conclusion: After thorough investigation and research my patient was diagnosed as case of auto immune hemolytic anaemia (cold Haemagglutinin disease). Pt.
